***Get ahead of the game and get your child's sports physical before the season starts***
Pro Tip: If your child has a doctor's visit or well child check up before July, bring the sports physical form to the visit and have the provider fill out the form. Make a copy for you and have another copy to turn in at onsite registration or at beginning of the season. Physical must have been completed after January 1st of the current year.
- Football Player/Cheerleader Packets must include:
- Player packet (printed off of the online registration portal)
- Birth Certificate (certified copy only) - Physical on the AYF Medical Clearance Form (make sure this is not the resume physical that comes with your packet)
- Copy of the player's insurance card
- Last report card received
